Complete Rehab of Lawn

Hi everyone! I have a yard I want to completely rehabilitate. The homestead, in New Hampshire, consists of two deep, skinny building lots split by a front to back driveway that leads to a rear garage. The house (facing it) is on the left side of the driveway and the right side is pretty much all yard with perimeter landscaping which includes 20' tall arbor vitae. The entire yard is in bad shape but for right now I would like to concentrate the approximate 5,000 square foot parcel that's on the right side of the driveway. There is a little grass left along the southwest edge of this plot but the rest is all crabgrass, weeds, and moss. I have not done a soil test as of yet. What would be everyone steps to rehab it?

We still have a 1' of snow on the ground so I can't get any yet. Think of the worse weedy yard with 2 blades of grass per square foot.
