What kind of things do you do to gain customer feedback?

What type of things don't you do that you feel you should?

I personally feel that quarterly or monthly surveys should be send out or monthly phone calls

I always send out a yearly survey. The only problem is that usually only about 1/4 of people return them, and those that return you can tell that they are not answering them honestly because they check off services that they are satisfied with, even though they don't receive those services.
