Proposal templates?

Hello all, I was just wondering if anyone has a template of a proposal they could post. *I have been given a wonderful opportunity to possibily get 12 yards from a local realestate developer and he had asked me to send him another bid b/c he will be wanting me to water them once a week. *I have an idea about how much i will be charging but just wanted to see if i could get a couple visuales to help me get an idea of how exactly it should look. *Thanks

And here is a worksheet I fill out when somone calls for an estimate. I fill the address and location out on the phone then fill the rest out when I go look at the property.
