Need some insight from you guys. I am getting ready to prepare a survey to email to my customers. It will be through my website and the customers who respond to them will remain anonymous.
I would like to see what would be some of the questions that you guys might ask your clients. Shoot me some ideas please. Thanks to all who respond!

here are a few thoughts, not sure that I would use them they are just the first things that came to mind.

are you happy with your lawn service? 1-10
what would you like to see done different?
are you considering changing lawn care providers?
what services would you like to see offered by your lawn service provider?
would you hire your lawn guy to clean your pool?
would you hire your lawn guy to clean your gutters?
would you hire your lawn guy to clean your etc...?
do you feel the price you are paying for lawn service is fair?

Have we done what was outlined in our marketing material?

How would you rate our staff?

How would you rate our service?

How satisfied are you with the overall value of services?

Would you recommend us to friends, family, neighbors, and colleagues?

May we use your comments on our website and or marketing material?

Did you check out the free survey section (http://www.gopherforum.com/showthread.php?t=6658) in the downloads area? There are a few surveys there that might help.

How will you operate your questionnaire? Will you email all your customers to visit your website and fill out the questions or what?

Thanks Steve, I have seen this before. One of the questionaires would be for a new or potential new customer and the other is for an exsisting customer.
I would like to get some more ideas if anyone has used a survey form before.

Thank you ProCut for the 2008 post from Steve. I had not seen that before.

How does the purpose for each of these groups differ?
